Disability application

Disability application for one of your patients.
The patient must satisfy the age and eligibility conditions to receive this benefit.

In order to receive a disability pension, the insured must satisfy the age and eligibility conditions.

If you believe that your patient's health prevents them from working full-time, you can complete an application for the granting of a "disability" pension and send it to the fund's Medical Department

 

You will receive a medical form that you must return to the Medical Advisor

The fund's Medical Advisors can also consider your patient's disability application if their health has stabilised after three years of work interruption. In this case, they will contact you and send you a medical disability application form that you must complete and return to them.

The insured will then be notified of the fund's decision, on the Medical Advisor's advice.
